Application of a multi-component exercise program in patients with major neurocognitive disorders

Objective To explore the application and effect of Vivifrail multi-component exercise intervention program in patients with major neurocognitive disorders. Methods In this quasi-experimental study,52 patients with major neurocognitive disorders who were hospitalized in a municipal psychiatric hospital from June 2021 to June 2022 were selected as research subjects and divided into Ward A as the intervention group(26 cases) and Ward B as the control group(26 cases). The control group was treated with routine nursing,and the intervention group was treated with 12-week Vivifrail multi-component exercise on the basis of routine nursing. Short Physical Performance Battery(SPPB),Timed "Up and Go" Test(TUGT),6 Minutes Walking Test(6MWT) and Chair Sit and Reach(CSR) were used to evaluate physical function in both groups before and after intervention. Results After 12 weeks of intervention,the total score of SPPB(t=-4.312,P<0.001) and its 3 test dimensions-balance test(t=-3.029,P=0.004),4 m walking speed test(t=-3.972,P<0.001),chair standing test(t=-2.912,P=0.005),and CSR test results (t=-3.446,P=0.001) were significantly different between the 2 groups. The results of repeated measures analysis of variance showed that all the indexes in the 2 groups had interaction effects(P<0.001). Except for the 4 m walking test of SPPB,the time effects of other indexes were statistically significant(P<0.05),and the variables with statistical significance of intergroup effect included balance test of SPPB and CSR test(P<0.05). Conclusion Vivifrail multicomponent exercise can significantly improve the balance function,walking speed,muscle strength and flexibility of patients with major neurocognitive disorders in medical institutions,but the improvement of body flexibility and heart and lung function was not obvious.

Construction of comprehensive nursing scheme of intestinal blind intubation for ischemic stroke patients

Objective The study aimed to construct a multidisciplinary,scientific,and standard comprehensive nursing scheme of intestinal blind intubation for patients with ischemic stroke.Methods Literature review,semi-structured interview,observational survey,and Delphi technique were used to construct the scheme. Semi-structured interview and observational survey were used from August to September 2020. 15 experts from clinical nursing,clinical medicine,and intensive medicine were consulted in 2 rounds of Delphi experts consultation from September to December 2020.Results After interviewing 20 experts and observing 7 cases,more than 100,000 words of textual materials and 325 minutes of videos were formed. After the analysis of materials,5 core categories and 18 categories were sorted out. 2 rounds of Delphi experts consultation were carried out,and the effective recovery rate of the questionnaire was 100%. The Delphi expert authority coefficients were 0.787 and 0.795,and the coordination coefficients were 0.112 and 0.367(P<0.01). The coefficient of variation of each item in the second round was 0~0.12,and the full score frequency was 20%~100%. Finally,the comprehensive nursing scheme was constructed,which included 5 first-level indicators (roles and functions,standard service,core operation specification,emergency plan,and mode implementation path),15 second-level indicators,and 43 third-level indicators.Conclusion The comprehensive nursing scheme for ischemic stroke patients was scientific,specific,and practical,which could provide references for the promotion and application of the technique of intestinal blind intubation.

Summary of the best evidence of artificial airway endotracheal tube cuff management in ICU adult patients

Objective To search,evaluate,and summarize the best evidence of endotracheal tube cuff management in adult ICU patients,and to provide a reference for clinical practice and standard formulation. Methods We searched UpToDate,BMJ Best Practice,Guidelines International Network(GIN),National Guideline Clearinghouse(NGC),Joanna Briggs Institute(JBI),Cochrane Library,PubMed,Embase,Web of Science,CINAHL,National Institute for Health and Care Excellence(NICE),China Clinical Guidelines Library,Yimaitong,Wanfang Database,China Biology Medicine(CBM),CNKI on artificial airway endotracheal tube cuff management in ICU adult patients,including clinical decision-making,guidelines,systematic reviews and expert consensuses published from January 2012 to February 2022. There were 4 researchers who evaluated the quality of the included articles and 2 researchers extracted evidence from the articles that met the quality standards. Results A total of 22 articles were included,including 3 clinical decision-makings,7 guidelines,3 expert consensuses,and 9 systematic reviews. 29 pieces of best evidence were summarized,including artificial airway evaluation,endotracheal tube cuff selection,cuff inflating method,cuff inflating time,cuff pressure regulation,cuff leakage evaluation,removal of secretions from the cuff,and cuff leakage test. Conclusion The best evidence of artificial airway endotracheal tube cuff management in ICU adult patients provides evidence-based evidence for clinical healthcare professionals. Scientific management of artificial airway endotracheal tube cuffs in ICU adults helps to reduce the incidence of ventilator-associated pneumonia.
